Assumption of Risk

I understand that participation in basketball training, drills, games, conditioning, strength exercises, and related athletic activities involves inherent risks, including but not limited to falls, collisions, sprains, strains, fractures, concussions, heat-related illnesses, and other injuries that may result in serious harm.

I voluntarily choose to participate (or allow my child to participate) and fully assume all risks associated with participation in the program.

Medical Fitness

I certify that I (or my child) am physically capable of participating in basketball-related activities and have disclosed any known medical conditions, injuries, or limitations that may affect participation.

Release of Liability

In consideration of being allowed to participate in the basketball training program, I, on behalf of myself, my child, my heirs, and representatives, release and hold harmless Next Gen Basketball Training, its owners, coaches, trainers, employees, volunteers, facility operators, and affiliates from any and all claims, liabilities, demands, damages, or causes of action arising from injury, illness, property damage, or loss that may occur during participation, except where prohibited by law.

Emergency Medical Care

In the event of an injury or medical emergency, I authorize program staff to obtain emergency medical treatment if necessary. I understand that all medical expenses are my responsibility.

Photo & Video Release

I grant permission for photographs and videos taken during training sessions to be used by Next Gen Basketball Training for marketing, promotional, and social media purposes without compensation.

Acknowledgment

By checking the box below, I acknowledge that I have read, understood, and voluntarily agree to the terms of this waiver and release of liability.
